Applicants must possess a B.Sc. (Nursing) or Post Basic B.Sc. (Nursing) with an integrated Certificate in Community Health for Nurses (CCHN) from an Indian Nursing Council or State Nursing Council-approved institution. Candidates are required to meet the age limits set, between 21 to 40 years as of the last application date. Successful candidates must also commit to a minimum three-year service period, formalized by a Surety Bond worth Rs. 2.50 Lakh on a non-judicial stamp paper of Rs. 100.
The selection process will include two phases:
Merit will be generated based on the performance in the CBT.
Candidates shortlisted based on the CBT will be invited for the DVP.
Candidates should ensure that they meet the eligibility criteria before filling the application. Provisional admission to Computer Based Test will be given; verification of documents will be carried out during DVP, if the candidates get shortlisted. All the uploaded documents at the time of online application have to be produced at the time of DVP. The Category candidates belonging to OBC (NCL) and EWS need to provide valid certificates issued on or after 01.04.2024. See point no. 4(d).
Note: Failure to pass the DVP will result in the cancellation of the candidate's application.
The Computer Based Test will be conducted in major cities across Uttar Pradesh. NHM reserves the right to assign any test city to the candidate.
The test duration is 2 hours (single sitting) and includes two sections, totaling 100 marks.
The test comprises 100 multiple-choice questions (MCQs), each worth 1 mark. No negative marking is applied for wrong answers, and no marks are given for unanswered questions. The question paper will be available in both English and Hindi, with the English version taking precedence in case of inconsistencies.

No application fee is required to apply for the Community Health Officer (CHO) position in NHM, UP.
